Anonymous reporting

Use this channel for safety issues, harassment or discrimination, payroll concerns, suspected fraud, retaliation, policy violations, or other conduct that does not feel right.

Emergency or immediate danger?

Call 911. For an imminent workplace safety hazard, you may also contact OSHA at 1-800-321-OSHA (6742).

Your identity is not required.

This form does not ask for your name, employee number, or work email. Avoid details that could identify you if you want to remain anonymous. Your report will be stored for CTSS review; as with any website, hosting providers may retain limited technical security logs.

CTSS whistleblower & non-retaliation policy

Your good-faith report is protected.

CTSS encourages employees to raise honest concerns about suspected unlawful, unsafe, unethical, or policy-violating conduct. CTSS prohibits retaliation against anyone who makes a good-faith report, asks a compliance question, refuses to participate in unlawful conduct, or cooperates in a workplace investigation.

What protection means

Retaliation—including threats, intimidation, reduced hours, unfavorable assignments, discipline, discharge, or other adverse treatment because of protected reporting—is prohibited. Anyone who believes retaliation has occurred should report it promptly through this channel or directly to Human Resources.

How reports are handled

CTSS will review reports promptly and fairly, limit information to people with a legitimate need to know, and protect confidentiality as far as reasonably practicable. Absolute confidentiality cannot be guaranteed when disclosure is necessary to investigate, respond to an immediate risk, provide due process, or comply with law.

Good-faith standard

You do not need proof before raising a concern. A good-faith report is one made with an honest belief that the information may show a problem. A report is still protected if an investigation does not substantiate it. Knowingly false statements are not good-faith reports.

Your outside reporting rights

This internal channel is optional and does not limit your right to contact OSHA, the EEOC, the U.S. Department of Labor, the NLRB, law enforcement, or another government agency. External claims may have short filing deadlines, so using this form does not pause or extend those deadlines.
